Erectile Dysfunction also known as ED is the consistent or recurrent ability to achieve or maintain a penile erection sufficient for sexual satisfaction.

 In studies done on Western populations, Erectile Dysfunction (ED) is present in 5% of men in their 40s, 10% of men in their 60s, 15% of men in their 70s and 40% of men in their 80s. In fact, it is estimated that 322 million men worldwide will be affected by Erectile Dysfunction (ED) by 2025, more than double the amount of men worldwide with Erectile Dysfunction (ED), a mere 20 years ago.

Erectile Dysfunction Signs & Symptoms

Erectile Dysfunction (ED) is unfortunately associated with increasing age, depression, obesity, lack of exercise, chronic diseases such as diabetes, blood pressure, cholesterol, heart disease and prostate problems. Besides affecting men and their quality of life, Erectile Dysfunction (ED) also affects the quality of life of their partners.

Studies have also shown that partners of men with Erectile Dysfunction (ED) experience lower sexual satisfaction in correlation to the severity of Erectile Dysfunction (ED) experienced by the men.

Because Erectile Dysfunction (ED) is becoming more common, more men end up not sharing their symptoms or concerns with their doctors, choosing to suffer in silence. Ultimately, leaving most cases of Erectile Dysfunction (ED) undiagnosed and untreated.

How Do I Know If I have Erectile Dysfunction

If you scored between 1-7 points, you likely have severe Erectile Dysfunction. 8-11 points moderate Erectile Dysfunction, 12-16 points are mild to moderate Erectile Dysfunction, 17-21 mild Erectile Dysfunction and lastly 21-25 points no Erectile Dysfunction.

What are all the causes of Erectile Dysfunction?

The erection mechanism is a series of complex processes between the vessels and the nerves where through certain pathways, chemicals are released to increase blood flow into the penile tissue thereby causing blood to remain within the penile tissue and therefore a resultant erection.


Aging is a risk factor leading to ED. Therefore,  many men often assume this is an unavoidable consequence of the aging process. However in a male aging study done in the US in 1994, 1 in 3 men in their 70s did notsuffer from ED. Yes that’s right, 1 in 3 min in their 70s had good erections!


However aging does predispose one to poorer caridovascular health, resulting in heart disease. Because a weak heart will mean poor blood flow, especially to the penis, therefore the risk factors that lead to heart diseases thus also apply to the risk of developing ED. These include lifestyle choices such as smoking, obesity, a sedentary lifestyle and even heavy alcohol use. These factors not only result in the likelihood of blood vessels getting obstructed, they are also postulated to contribute to low testosterone,  thyroid hormone level changes and the impairment of blood vessel function; all of which lead to ED as well.


Furthermore, as we were describing earlier, Diabetes, Hypertension, High cholesterol and depression all contribute to one’s risk of developing Erectile Dysfunction by more than 20-42% increased risk respectively. Also alarmingly, medication side effects from the treatment of these chronic conditions also resultanty lead to Erectile Dysfunction and account for 25% of all the Erectile Dysfunction cases.


Besides these vasculogenic, endocrine and drug causes of Erectile Dysfunction, on rare occasions, some people have conditions which affect their nerves such as a spinal cord injury, multiple sclerosis etc. With these nerves affected, they thus develop Erectile Dysfunction.


Direct damage or injury to the penis itself can also cause Erectile Dysfunction, although this is rare. But conditions such as Peyronie's disease where a calcified plaque causes the penis to be bent can be treated with medications or even shockwave therapy.


Last but not least, psychological causes are the final group of conditions which cause Erectile Dysfunction. These psychological causes often include a degree of anxiety. Whether it is the anxiety to perform, relationship problems or even depression and guilt.

What are the treatments available? - Erectile Dysfunction Treatment

There are numerous treatment options for Erectile Dysfunction. These range from oral medications to penis injection therapy or a combination of sorts, and now a more sophisticated type of treatment called Electro Shockwave Therapy (ESWT). Whilst this fancy sci-fi sounding treatment may sound complicated, it is actually painless, quick (15-20mins) and effective!


Previously, most patients would think that oral medication is also known as Phosphodiesterase 5 Inhibitors (PDE5 i) are good to medications and “cure” to achieve a chemical induced erection.


However, since 2010, the studies on ESWT have shown numerous benefits on the treatment of Erectile Dysfunction. The technical way of how this works, is basically an intense wave of energy passing through the penile tissue resulting in an inflammatory response to increase the blood flow and regeneration of tissue. This leads to 2 further biological responses that correct and repair the erection mechanism. Mainly, new blood vessel formation which thus allows more blood flow into the penis and also rejuvenation of the penis tissue by allowing the body to repair a previous veno-occlusive function of the smooth tissue, to allow blood to be trapped in the penis and ultimately a prolonged erection.


So doctor, what do all these scientific jargon mean for me as a patient? Hold your horses (or horse) I am coming to that right now!


At the clinical level, patients who previously depended on oral medication were able to stop depending on to achieve an erection and patients with severe Erectile Dysfunction where oral medications did not work, started to see oral medications become effective for them again and the frequency of oral medications or even injection therapy has decreased.

ESWT gives most men a “natural” erection, decreasing their dependence on oral or injectable therapy, or for those men with severe ED, to start to have a response to these first and second line modalities again.


Many trials have shown the benefits of ESWT on Erectile Dysfunction, furthermore for men without Erectile Dysfunction, ESWT was also shown to improve erections or prevent Erectile Dysfunction. In our experience, patients see effects after 1 session, but prolonged benefits are usually seen after 6 treatment sessions.
