Erectile Dysfunction (ED) Singapore: Symptoms, Causes & Treatment

Erectile Dysfunction Treatment Singapore

Can’t get it up? Don’t worry – Erectile dysfunction (ED) is a relatively common condition amongst men and you’re not alone. Most cases of ED may be managed effectively with the right course of treatment.

Since the 1990’s, erectile dysfunction (ED) has been gaining greater recognition as a common and significant problem for men and their sexual partners. Higher rates of men seeking help are being driven by the emergence of more accessible and effective treatment options, especially in primary healthcare provider settings.

Studies indicate that up to 20% of sexually active men suffer from some degree of erectile dysfunction.

The prevalence of ED increases with age, with up to 86% of men aged 80 years or above unable to achieve a satisfactory erection.

What is Erectile Dysfunction

Erectile dysfunction (ED) refers to the persistent inability to achieve or maintain an erection sufficient for satisfactory sexual activity.

While occasional difficulty with erections is not uncommon, ED is typically diagnosed when the issue occurs consistently over time. It may develop gradually or occur suddenly, depending on the underlying cause.

ED is often linked to blood flow, nerve function, hormone levels, or psychological factors. In some cases, it may also be an early indicator of underlying health conditions such as cardiovascular disease or diabetes.

What are the Symptoms of Erectile Dysfunction

The symptoms of erectile dysfunction can vary in severity and frequency, but commonly include:

  • Difficulty achieving an erection
  • Trouble maintaining an erection during sexual activity
  • Reduced firmness of erections
  • Decreased sexual desire (libido)
  • Erections that are less frequent or inconsistent
  • Increased anxiety or stress related to sexual performance

If these symptoms persist over time, it may indicate an underlying medical or psychological condition, as seen below.

Causes of Erectile Dysfunction

Erectile dysfunction is a complex condition which can be caused or associated with multiple factors and chronic diseases, such as:

  • Venous leak (cavernosal insufficiency)
  • Obesity
  • Medication side effects

While erectile dysfunction is not a life-threatening condition, it can often be the first sign of a condition like heart disease or diabetes. Furthermore, it can have a considerable impact on a man’s sense of well-being, relationships, and quality of life.

Different Types of Erectile Dysfunction

This is how ED is typically classified, based on the different causes.

  1. Vasculogenic Erectile Dysfunction
This is the most common type and is caused by reduced blood flow to the penis. It is often associated with conditions such as hypertension, diabetes and cardiovascular disease.
  2. Neurogenic Erectile Dysfunction
This occurs when there is disruption to the nerve signals responsible for triggering an erection. It may be linked to neurological conditions, spinal cord injuries, or nerve damage from surgery.
  3. Hormonal Erectile Dysfunction
Low testosterone or other hormonal imbalances can affect libido and erectile function.
  4. Psychogenic Erectile Dysfunction
Psychological factors such as stress, anxiety, depression, or relationship issues can interfere with sexual performance. This type may occur suddenly and can vary depending on the situation.
  5. Medication-Induced Erectile Dysfunction
Certain medications, including those used to treat high blood pressure or depression, may contribute to ED as a side effect.

In many cases, ED is multifactorial, meaning more than one of these factors may be involved.

Diagnosis of Erectile Dysfunction

Here’s how doctors typically diagnose ED.

  1. Medical and Sexual History
Your doctor will ask about your symptoms, lifestyle habits, and any existing medical conditions. This helps identify potential contributing factors such as chronic illness, medication use, or psychological stress.
  2. Physical Examination
A focused examination may be performed to assess the genitals, blood pressure, and signs of hormonal imbalance.
  3. Blood Tests
Blood tests may be used to check for conditions such as diabetes, high cholesterol, or low testosterone levels.
  4. Specialised Tests (if required)
In some cases, additional tests such as penile Doppler ultrasound may be recommended to evaluate blood flow to the penis.
  5. Psychological Assessment

If a psychological cause is suspected, your doctor may explore factors such as stress, anxiety, or depression.

Erectile Dysfunction Treatment Options

If you think you may have Erectile Dysfunction, the first step is to speak to your doctor.

There are many safe and effective treatments now available for Erectile Dysfunction, but any specific or recommended erectile dysfunction treatment may vary depending on the underlying cause, age and comorbid conditions such as heart disease.

Sometimes, simple lifestyle changes will benefit, such as losing weight, quitting smoking, and cutting down on alcohol. If the Erectile Dysfunction is being caused by a medication, you may be counselled to reduce the dose or change to an alternative drug.

Other Effective Erectile Dysfunction

Treatment Options Include:

  • Counselling
  • Oral medications (ED pills)
  • Injectables
  • Extracorporeal Shockwave Therapy (ESWT)
  • Vacuum pumps
  • Surgery (prosthesis)

Oral Medications (Including ED Pills)

The most common form of medications available are known as PDE5 inhibitors. This class of medications work by increasing blood flow to penile tissues and are effective for over 80% of patients.

Some possible side effects can include a headache, nasal stuffiness, feeling flushed, and slight muscle ache.

If you have heart disease and are taking a class of drug called nitrates, you will be advised by the doctor not to take them concurrently as this can potentially cause a significant drop in blood pressure.

It is also dangerous to mix the blue pills and alkyl nitrites together.

Please also inform your doctor about any other medications you are taking, including over the counter medications or herbal supplements.

Injectables

This is a pre-filled injection pen that is self-administered by patients into the penile tissue to achieve an erection rapidly and consistently.

As it is injected directly into the penis, there are fewer potential systemic side effects, which some people may experience with the oral medications.

Using this injectable, an erection can be attained within minutes of administration and usually will last up to one hour. Your doctor will advise whether this is a suitable treatment for you, as well as demonstrate self-injection technique.

While it may seem slightly daunting initially, most patients quickly get used to using this injectable and may find it more effective than just the oral medications.

Counselling

Erectile dysfunction can sometimes be caused by stress or anxiety, sometimes in combination with other medical conditions. These may be triggered by problems at work, family, or with your partner.

It can be useful to speak to a licensed therapist who can address these issues and reduce sexual anxiety, building confidence and improving aspects of your relationship.

This is a novel mode of treatment for Erectile Dysfunction which uses low-intensity shock waves directly to penile tissues to help stimulate the growth of new blood vessels.

It has been shown to be safe, painless, and effective for the treatment of vasculogenic Erectile Dysfunction (caused by blood vessel problems such as in people with diabetes and high blood pressure).

While the medications above help to treat the symptoms of erectile dysfunction, ESWT is the only method that aims to restore the natural mechanism to enable normal and spontaneous erections.

It has also been shown to improve Peyronie’s disease in certain cases, by breaking up the scar tissue that is causing the abnormal penile curvature.

ESWT usually comprises of 6-12 sessions over a period of a few weeks, and results have shown to last for upwards of a year.

It is not effective for all types of Erectile Dysfunction, so make sure you speak to your doctor to decide if it is suitable for you.

Penile Pumps & Surgery

Penile pumps are vacuum devices which help increase blood flow to the penis and are sometimes used after prostate surgery to help with erectile rehabilitation.

You will be typically recommended a daily regimen designed to restore normal blood flow to the penis, but this program can sometimes take a few months to show improvement. If all other methods fail, your doctor may refer you for surgery with a urologist.

This usually involves the insertion of a penile prosthesis (implant), which can either be malleable (bendable) or operated with an inflatable pump.

Implantation of these prostheses usually occurs under general anaesthesia, and complications can include breakage or infection.

Erectile Dysfunction Prevention

While not all cases of erectile dysfunction can be prevented, especially those linked to ageing or certain medical conditions, many risk factors can be managed through lifestyle changes and consistent medical care.

  1. Manage Chronic Health Conditions
Conditions such as diabetes, hypertension, and high cholesterol can affect blood flow and nerve function. Keeping these under control with regular check-ups and treatment may reduce the risk of ED.
  2. Maintain a Healthy Weight
Excess body weight is associated with reduced testosterone levels and poorer vascular health. Gradual weight loss through diet and exercise can support erectile function.
  3. Exercise Regularly
Physical activity helps improve blood circulation, cardiovascular health, and hormone balance. Even moderate exercise, such as brisk walking, can be beneficial.
  4. Quit Smoking
Smoking damages blood vessels and restricts blood flow, which can contribute to ED. Stopping smoking may improve both vascular health and erectile function over time.
  5. Limit Alcohol Intake
Excessive alcohol consumption can interfere with nerve signals and hormone levels. Stopping or drinking in moderation may help reduce this risk.
  6. Manage Stress and Mental Health
Psychological factors such as stress, anxiety, and depression can play a role in ED. Addressing these early, whether through lifestyle changes or professional support, can help maintain sexual function.
  7. Review Medications with Your Doctor
If you are on long-term medications, it may be helpful to review potential side effects with your doctor. In some cases, alternatives may be available.

Taking a proactive approach to overall health not only supports erectile function but also reduces the risk of more serious underlying conditions.

Frequently Asked Questions

1. Is there a permanent cure for erectile dysfunction?
This depends on the underlying cause. In some cases—such as those related to lifestyle factors or certain medical conditions—erectile dysfunction may improve significantly with treatment and lifestyle changes. However, for others, ongoing management may be needed rather than a one-time cure.

2. What is the main cause of erectile dysfunction?
There is no single main cause. Erectile dysfunction is often linked to reduced blood flow, which may be associated with conditions like diabetes, high blood pressure, or cardiovascular disease. Psychological factors such as stress and anxiety can also play a role.

3. Is erectile dysfunction permanent?
Not always. Many men experience temporary erectile dysfunction due to stress, fatigue, or lifestyle factors. With appropriate treatment and management of underlying causes, erectile function can often improve.

4. Does masturbation cause erectile dysfunction?
Masturbation does not cause erectile dysfunction. However, factors such as excessive frequency or reliance on specific stimuli may affect sexual response in some individuals. If there are concerns, it is best to discuss them with a doctor.

5. How to know if I have erectile dysfunction, can I test it?
Erectile dysfunction is usually identified based on symptoms, such as difficulty achieving or maintaining an erection consistently. A doctor may assess your medical history and, if needed, perform blood tests or other evaluations to determine the cause.
