What is Andropause (Men Menopause)?
There will come a point in our lives when sex hormones in our body undergo a gradual decline in production as one age. Whilst this is known as menopause in women, it is often known as Andropause in men.
Andropause starts in the 4thdecade of life with about 5% of men affected and rises to 30% and 45% of men in their 50s and 60s respectively. This number exponentially increases to 70% and 90% for men in their 70s and 80s respectively as well. However, it is also not unusual for men to experience andropause, even in their thirties.

Could I be having andropause?
The symptoms experienced during Andropause progress gradually
Initially, as testosterone levels are low,
- men would often feel tired, more so at the end of the day
- sexual desire and libido would also start to wane
- feelings of unexplained sadness, depression is experienced
As andropause worsens with persistent low testosterone levels, physical symptoms before more apparent which include
- a decrease in muscle bulk
- a decrease in bone strength
- loss of facial and or body hair
- the growth of breasts, also known as gynecomastia

How do I get help?
At Dr Tan and Partners, we believe in a multidisciplinary and holistic approach to supporting your men’s health journey.
We adopt a three-pronged approach where we start with blood tests to examine your overall health. These tests would give us an indication of the presence of any of the metabolic syndrome components.
We would then explore with you treatment options for any discovered chronic conditions. These are usually treated with medication and monitored on a 3-6 month interval.
Conditions such as
- Hyperlipidemia (high cholesterol)
- Fatty liver
We will also work with you and tailor an exercise prescription to your needs coupled with medical adjuncts to help you lose weight.
What treatment is there specifically for Andropause?
If testosterone replacement is indicated and started, the 2 commonly used approaches are either as an injection into the muscle once every 3-6 months or as a gel daily for 3-6 months as well.
Men who use testosterone replacement gels must wash their hands after putting on the medicine. That's because testosterone can get on other people's skin. It can cause harmful side effects, especially in children. Gels must be put on skin that is covered by clothes. This can help keep testosterone from getting on other people.

As with any treatment that you are started on, there will be a discussion of the benefits and the disadvantages with you and if treatment is initiated. We will closely follow up with you through regular exams and blood tests.
Further considerations for Testosterone (Androgen) replacement therapy are that it is appropriate in men with cardiovascular disease or risk of cardiovascular disease. Men who have been successfully treated for prostate cancer may also be candidates but will require closer monitoring.
What happens after I start testosterone replacement therapy?
Are there any Follow up
Our doctors will check in on you, usually after 3 months to discuss any improvements to your overall symptoms, namely libido, energy levels and mood.
If you do not notice a difference, we would then measure your testosterone again. If levels remain low, we would explore challenges of the treatment with you (if on a daily gel regime) or a change in dose or route of administration.
If your testosterone levels on recheck are acceptable, we will explore other causes that may contribute to your symptoms as well.
As starting any new medication, your safety is our utmost importance. We will most likely stop your treatment if you experience any adverse events not relating to the dose or route of administration. If there are side effects relating to the dose or administration, we will adjust it accordingly to find the balance for you.
If there are no side effects experienced, we will continue to closely monitor you for a ongoing symptom improvement and or any new side effects.
